What Our Privacy Policy Covers
This Privacy Policy applies any and all personal information that TRUE
CARE receives from you and stores in our company’s database.“Personal information” is information about you such as your name, street address, email address, telephone number, and other similar information that is received from you by
TRUE CARE.
This Privacy Policy does not apply to the practices of individuals or
companies that TRUE CARE does not own or control or to individuals or other
business entities that TRUE CARE does not employ and manage.
Information Collection and Use
TRUE CARE collects personal information when you register or join with
TRUE CARE, when you use TRUE CARE products or services, and when you visit TRUE
CARE web pages.
When you join our plan, we request information such as your name, street address, email address, birth date, gender,
and zip code. Once you register with
TRUE CARE and use our services, you are no longer anonymous to us.
TRUE CARE also automatically receives and records information on our server logs from your browser, including your IP address,
TRUE CARE cookie information, and the page you request. "Cookies" are small pieces of information that are stored by an Internet User's browser on the Internet User's hard drive.
TRUE CARE uses information for the following general purposes: to customize the advertising and content you see, fulfill your requests for products and services, improve our customer services, contact you, conduct research, and to provide anonymous reporting for internal and external clients.
Information Sharing and Disclosure
TRUE CARE does not rent, sell, or share personal identifying information about you with nonaffiliated individuals or companies except where it is necessary to provide the information to acquire the products or services that you've requested and where we have your permission to do so;
or we also share your personal identifying information in response to subpoenas, court orders, or other legal process, and/or to establish and/or exercise our legal rights and/or to defend against legal claims, and we will also share your information at such times and under such circumstances that we determine, in our sole discretion, that it is necessary to share the information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of the
TRUE CARE Terms of Use, or as otherwise required by law; and we will transfer information about you if
TRUE CARE is acquired by or merged with another company, and in this event,
TRUE CARE will notify you before information about you is transferred and becomes subject to a different privacy policy.
Cookies
TRUE CARE may set and access TRUE CARE cookies on your computer. Advertisers or other companies do not have access to
TRUE CARE's cookies. TRUE CARE uses web beacons to access TRUE CARE cookies inside and outside of our network of web sites and in connection with
TRUE CARE products and services.
Confidentiality and Security
We limit access to the personal information about you within our company to
only the employees who we believe reasonably need to come into contact with that information in order to provide products or services to you or in order to do their jobs. We have physical, electronic, and procedural safeguards that comply with federal regulations to protect personal information about you. In

CARE uses industry-standard SSL encryption to protect data transmissions.
Changes to this Privacy Policy
TRUE CARE may from time to time update this policy. We will notify you about any significant changes in the way we treat personal information by sending you a notice to the primary email address specified in your
TRUE CARE account or by placing a prominent notice on our site.
This
plan is NOT insurance. This plan
provides discounts at certain healthcare providers for medical
services. This plan does not
make payments directly to the providers of medical services.
The plan member is obligated to pay for all healthcare
services but will receive a discount from those healthcare providers
who have contracted with the discount plan organization.

This discount card program
contains a 30 day cancellation period. AR and TN Residents:
receive a full refund if membership is cancelled within the first 30
days. MD, ND, OK, SC, TX and SD residents: Member shall receive a full
refund of membership fees, excluding registration fee, if membership
is cancelled within the first 30 days after receipt of membership
materials. All other members receive a refund of all fees excluding the
$25 Application fee. The discount medical card program makes
available, before purchase and upon request, a list of program
providers, including the name, city, state, and specialty of each
program provider located in the cardholder's service area.

The range of discounts for medical
or ancillary services provided under the plan will vary depending on
the type of provider and medical or ancillary service received.
